What do we do?

For the residents:

  • We support your named General Practitioner with all administrative tasks
  • Support is provided so your doctor can visit you on a weekly basis, mostly on a set day
  • Most doctors are also contactable most evenings till a certain time (your doctor shall discuss and explain this further personally)
  • Out-of-hour emergency doctor/ locum GP service is provided
  • Provision for your doctor to send medication scripts electronically to you or directly to the Pharmacy.

For the facility:

  • We work with doctors to establish clear and timely communication channels detailing resident’s care
  • Aim to provide support to GP so can visit at a time suitable for the resident and the facility
  • Our processes and support allow most doctors to work independently reducing any burden on the facility’s nursing teams
  • We assist doctors in completing necessary administrative tasks concerning residents in the facility
  • Discuss with nursing staff and formulate SMART goals for resident care

The above services allow your GP to:

  • Focus on residents’ holistic care
  • Be contactable via mobile or email (as per prior arrangement with GP)
  • Be available to discuss care with the resident’s family
  • Conduct a telehealth consultation, if and when needed
  • Take part in case conference meetings with family and nursing teams
  • Conduct timely comprehensive medical assessments
  • Conduct annual residential medication management reviews (RMMR)
  • Refer to comprehensive geriatrician assessments or to medical specialists, when needed
  • Provide a doctor or nurse-managed vaccination service (your doctor shall discuss your suitability)
  • Review of ‘as needed’ medications and minimize polypharmacy.
